Permission Slips
The Youth Ministry offers allows you to download & print permission slips and registration forms directly from this website.
Most Permission Slips require Adobe Acrobat Reader. This software comes already installed on most computers. If you are having trouble opening up a permission slip then you probably need Adobe Acrobat Reader. Click on the link / picture below to get your free copy of Adobe Reader.
All permission slips are also available at the parish office.
ALL PERMISSION SLIPS ARE TO BE RETURNED TO THE PARISH OFFICE.
We must have a Registration Form for every participant of the youth ministry. Be sure to return your registration form at the beginning of each year. You can download and print the registration form below. This must be returned before you are able to attend any youth ministry events.
 Winter Extreme is one of our most popular events and we have only a limited number of reservations we can accept. RESPOND EARLY.
Deposits are being accepted now. Permission Slip must accompany your deposit and it is non-refundable.
